In Russia, the fall of 2012 was quite busy with medical events, where the issues and problematic of medical rehabilitation and prevention were discussed.

In Russian Federation, the fall of 2012 was quite busy with medical events, where the issues and problematic of medical rehabilitation and prevention were discussed. I took part in some of them.

On September 24-26, Congress “Rehabilitation and Medical Spa 2012. Rehabilitation of Patients with Somatic Diseases” was held in Moscow, Russia. It was organized by the Ministry of Public Health of Russian Federation. The congress was attended by hundreds of professionals in the field of physical therapy and rehabilitation, doctors of different specialties (pediatricians, cardiologists, neurologists, pulmonologists, etc.) coming from many regions of Russian Federation.

I have made two reports: J. A. Nakatis, A. V. Chervinskaya “Actual problems of early detection and rehabilitation of patients with pulmonary diseases”. The report reflects the importance of early detection of chronic lung disease and occupational diseases, and the need for timely preventive and rehabilitation measures. We presented the algorithm of “Lung Health Program”, which is implemented in a large medical facility – Clinical Hospital № 122 named after Sokolov of Federal Medical-Biological Agency of Russia. The following capabilities of complex approaches, which apply physical factors, were considered: controlled halotherapy in salt room “Aeromed”; individual halotherapy using desktop halo-inhaler; bio-controlled air-ion therapy; inhalation physiotherapy; dosage oxygen therapy; usage of portable breathing aids; non-invasive lungs ventilation (CPAP, BiPaP); etc.

A. V. Chervinskaya “Relevant aspects of halotherapy in medical rehabilitation” Long-term experience in developing and application of saline microclimate indoors (in salt rooms and halo-chambers) allowed the significant improving the method of halotherapy and the equipment for its implementation. Since 1995, the innovative medical technology of halotherapy is being used – controlled halotherapy. Controlled halotherapy provides differential dosing of dry salt aerosol and constant control of its concentration level during the procedure, which improves the efficiency and safety of treatment and optimizes the terms of its indication. Possibility of selecting of dry salt aerosol concentration ensures adaptation of controlled halotherapy to various areas of medical rehabilitation and health improvement. The innovative technology allows for continuous automatic control and management of parameters of the microclimate and the halotherapy facility in whole. It is done using software-electronic system based on the interface with included optic-electronic indicator of salt aerosol concentration, specialized microcontroller, multifunction touch-control panel, and software system HaloCONTROL®.

On October 1-3, in southern Russia in Sochi The 4th International Congress “Health resort improvement, treatment and rehabilitation of patients with social important and occupational diseases” took place in clinical sanatorium complex “AquaLoo”. The Congress was organized by the Federal Medical-Biological Agency of Russia. Under the guidance of the State Scientific Center of the Russian Federation “Federal Medical Biophysical Center named after A. I. Burnazyan”, the leading research institutions of Russia in the field of rehabilitation and sanatorium treatment have taken part in the Congress: Pyatigorsk State Research Institute of Balneology of the Federal Medical and Biological Agency, Research Center of Balneology and Rehabilitation, Tomsk Research Institute of Balneology and Physiotherapy of the Federal Medical and Biological Agency, Russian Scientific Center for Restorative Medicine and Balneology of the Russian Ministry of Public Health.

I have made the report: A. V. Chervinskaya “Topical aspects of pulmonary rehabilitation in health resort care” The report reflected the topicality of the problem of respiratory diseases, their social-economic importance, the increase in respiratory rehabilitation needs, the need and importance of sanatorium care. A review of modern methods of respiratory rehabilitation, mechanisms of their action was considered. A special role was given to the methods, which use inhaled physical factors – controlled halotherapy, bio-controlled air-ion therapy, and hardware controlled aromatherapy. Scientific data of controlled halotherapy and other methods efficacy for different respiratory diseases in adults and children were presented.

On October 18-19, in St. Petersburg hosted the Congress of Physicians Physiotherapists “Physiotherapy – Yesterday, Today, and Tomorrow» dedicated to 125th anniversary of the Department of Physiotherapy and Medical Rehabilitation of North-West Medical University named after I. I. Mechnikov. The Congress was organized by the Public Health Committee of St. Petersburg, North-West Branch of the Russian Academy of Medical Sciences (RAMS), North-West State Medical University named after I. I. Mechnikov, North-West Physiotherapy Association, and Association of North-West Resorts. The Congress was attended by hundreds of doctors and scientists from different parts of Russia and the Baltic countries. There were considerable debates about the role of physical therapy and physical therapists doctors in medical rehabilitation. The undoubted importance of physical therapy in the prevention and rehabilitation and the need of the development of this branch in science and medical practice were stressed. I participated in the Congress as a speaker and moderator of one of the workshops.

The 22nd Annual Congress of ERS

22nd Annual Congress of the European Respiratory Society was held in Vienna, Austria on 1-5 September, 2012

In September 2012 I participated in the 22nd Annual Congress of the European Respiratory Society. This year it was held in Vienna, Austria on 1-5 September with over 12,000 doctors and specialists in respiratory medicine attended.

Dozens of symposia, presentations, and discussions were held daily. The most topical issues of diagnosis, treatment, prevention, and rehabilitation of pulmonary medicine were presented and discussed. One of those issues is PR COPD, and many studies were devoted to it. Definition of PR COPD was adopted in 2006 by ATS/ERS Group.

We got to know more about COPD in the last years: about its complex nature, about systemic manifestations of the disease, and about psycho-emotional status of patients. Representation of the goals and purposes of PR COPD is therefore changing — more and more attention is now given to the custom (i.e. personalized) approach. One of hot topics of Congress was presentation and discussion on the new definition of PR COPD, which will be adopted by ATS/ERS in the near future. Much attention was given to various methods and techniques of PR COPD — there is a constant search for new and effective methods help pulmonary patients.

The results of our research “Dry sodium chloride aerosol in rehabilitation of patients with COPD”, implemented jointly with Polish colleagues were presented at session “The latest insights in pulmonary rehabilitation”. In rehabilitation clinic of a resort in Poland we have conducted a randomized placebo-study and got a positive effect of dry salt of sodium chloride in the method of controlled halotherapy in patients with mild to moderate COPD. In the last 2-3 years, there has been a great interest toward “salt therapy” in Italy; significant amount of “salt rooms” has been built. Many owners and operators of “salt rooms” claim to practice “halotherapy”. The term “halotherapy” has become quite trendy to exploit, but what they mean by that? It turns out to be either plain sitting in the room with salt walls but practically without any salt aerosol in the air; or usage of wide range of saline nebulizers; or application of halogenerators not having necessary permits; or usage of some kind of “special salt-mixtures”, and even more than that. For all that, such owners declare to successfully treat asthma and many other serious illnesses in their “salt rooms”.

Such situation compromises method of halotherapy, and gives a rise to sharp criticism of it in medical circles. For regulation of the method, which is relatively new in Italy, Ministry of Health started carrying out expert examination of using salt rooms, and determination of halotherapy’s role in healthcare and health-improvement (benessere) system.

Conclusion of the reference:

Thus, at present, there are many different types of salt therapy, and many are called halotherapy. The fashionable trend and the lack of reliable information on halotherapy have led to the commercial spread of techniques, often based on pseudo-scientific information. Of the hundreds of salt rooms built in Europe, only a few have modern equipment for full, effective, and safe use of the method of controlled halotherapy. This situation hinders the advancement of halotherapy, and arouses distrust in the medical community to this intrinsically efficient method.[break] [break]
Simultaneously, it is my belief that based on already available substantial scientific research and clinical experience, halotherapy has great potential for the use medical practice. This method can become an effective means of respiratory hygiene in the rehabilitative and preventative respiratory care. There is great potential for the use of halotherapy for family, and, especially, for children’s rehabilitation.[break] [break]
Due to the possibility of a differentiated approach, controlled halotherapy has the potential to be used in the health resort and SPA industries.[break] [break]
There is an urgent need for accumulation and analysis of practical experience of halotherapy application in Europe.

We have discussed problem of regulating of application of salt rooms and halotherapy with one of heads of Ministry of Health departments.

The 38th World Congress of ISMH

The 38th World Congress of ISMH, the International Scientific Meeting of Medical Hydrology Balneology and Climatology, took place in Lanjarón-Granada Spain, 20-23 June 2012.

The 38th World Congress of ISMH, the International Scientific Meeting of Medical Hydrology Balneology and Climatology, took place in Granada – Spain, Lanjarón, 20-23 June 2012. The purpose of the International Society of Medical Hydrology and Climatology (ISMH ) is to design, plan, and co-ordinate scientific research in the fields of Health Resort Medicine and Spa Therapy for prevention, treatment, and rehabilitation programs.

Since 2008 Prof. Dr. M. Zeki Karagülle (Department of Medical Ecology and Hydroclimatology of Istanbul Medical Faculty, Istanbul University) is the president of Congress.

I am an active member of ISMH since 2005. I had participated in Congresses of ISMH in Istanbul (2006), Porto (2008), and Paris (2010), where I had presented following reports and publications: “Respiratory Hygiene in Health Resort Medicine”  61 KB , “Halotherapy in health resort medicine”  2.3 MB , and “Efficacy of halotherapy in asthma patients”  1 MB .

The motto of the 38th Congress «Medical Hydrology and Balneology: Environmental Aspects» reflects the importance of the preservation of the natural balneotherapeutic factors; water, mud, climate in general and the “healing environment” of the traditional spa resorts in particular. The President of 38th World Congress of ISMH was Professor F. Maraver (Spain).

Hundreds of professionals from the most of European countries, and also from USA, Canada, Turkey, and Japan participated in Congress.

Main topics of the Congress covered all aspects of common field, and were comprehensively presented as up-to-date lectures by the invited expert speakers. I was the member of Scientific International Committee and the chairman of  “New Trends” symposium.

I had presented the lecture “Salt Rooms and Halotherapy in European Health Resorts and Spas: Fashionable Trend or Real Therapy?”  1.7 MB and the report “Halotherapy in Rehabilitation of Patients with Chronic Obstructive Pulmonary Disease”  1 MB .

The report was prepared in cooperation with my colleagues from Poland, who were leaded by Prof. Irena Ponikowska, and had presented the result of our joint project of studying efficiency of halotherapy in patients with COPD. All the research work was carried out in balneological clinic  headed by prof. Ponikowska in Ciechocinek, Poland.

During my stay in the U.S., I visited several salt rooms, which are providing services for recovery. More and more people in the United States are interested in using halotherapy, which came to the U.S. from Russia and Europe.

Salt rooms where special equipment is installed – halogenerators for generating healing environment of superfine dry aerosol of sodium chloride – are gaining popularity.

On June, 14 in Johnson City, NY at the Salt Sanctuary at the “Spa at Traditions” the workshop for all the people interested in halotherapy was held.

Halotherapy workshop was organized by  Matthew Walsh (General Manager of the Salt Sanctuary), who invited me as a speaker.

I gave a lecture «Mechanisms of Action and Possible Therapeutic Applications of Controlled Halotherapy».

The keypoints of my lecture were: history of speleotherapy and halotherapy; mechanisms of action of dry salt aerosol of sodium chloride; researches and medical studies held in Russia and Europe; ways of creating the healing aerosol environment; advantages and benefits of  controlled halotherapy; application of controlled halotherapy in various fields of medicine and health improvement. The lecture aroused keen interest of the audience.
